I would like to start a registry of people who have had the misfortune of the axle shaft coming disconnected. This will give us some back up when the dealerships give lame excusses for the cause. Hopefully this problem does not cause an accident. Please also fill out the NHTSA report.
Here is the link to file a complaint: http://www-odi.nhtsa.dot.gov/ivoq/

VIN/Serial # - 223675
Build Date - Jan o4
Trans Type - M12 Manual
Date Of Mishap - July 2, 2004
Speed At Time Of Mishap - 20 MPH
Circumstances - Shifting From 1st to 2nd, Normal Driving
Vehicle Mileage At Mishap - 3100
City And State Of Mishap - Scottsdale, AZ
Bolts And Retainers Recovered At The Scene? - YES
Excuse That Dealer Gave - Aftermarket Tailpipe
Covered Under Warranty? - Yes ("as a favor")
Turn Around Time To Repair - 5 Business Days
Which Axle Connection Failed? - Inboard Driver's Side
Parts Replaced - Inner CV Boot Assy, Output Hub, Fasteners
Had You Experienced Severe Wheel Hop Prior To The Mishap? - Yes, 3 weeks prior
Additional Information Regarding The Mishap - Vehicle stopped accelerating on the 1-2 shift heard a slight clunk in the rear, towed to nearest dealership. Recovered bolts and retainers the next day.
Additional Information Regarding The Repair - Severely Harrassed by two dealerships, almost had to repair myself. First dealership dented the hood and right fender. Selling Dealership repaired under "limited" warranty as a favor, they would not provide a loaner. I had to ask the sales manager for help, they gave me a 24 hour test vehicle for the week. This was the 4th of July Weekend!

Please Use the other threads for discussion - I would like to leave this as a quick reference that people can fill out and print out as needed.

Additional Information Regarding The Repair - After finding out that the dealership has their head so far in their ass that they can't read a parts order form to find out which parts were in and which ones were on order, I contacted GM Executive services. They found out all the info and told me when to expect my car back (something the dealership still has no clue about). Al, the service guy at Ultimate Pontiac, tried to tell me that the 2 golf bags in the trunk caused the problem. I told him that was the most rediculous thing I had ever heard. No problems getting them to fix it though, just waiting on parts.

Thought I would make a blank for people

For you lazy :gr_jest: people, here's a blank of the form...The bolds just apply to each section - they won't affect what you type in. And I added a mods category at the end (more information = better, even though some people think I'm nuts for suggesting it).
